Gifted Iris Healthcare is seeking a compassionate and clinically skilled Registered Nurse to provide Developmental Disabilities Nursing Services (DNS) for an individual client in the Portland metro area. This role delivers hands-on skilled nursing care in a home setting, following an individualized care plan and clinical protocols, while working closely with our Clinical Director and care team to ensure the highest standard of safety and quality of life for the client.

Responsibilities:
Deliver skilled nursing care per the client's individualized DNS care plan (OAR 411-380-0030 standards)
Administer medications and treatments as ordered
Monitor and manage clinical conditions, including G-tube care, seizure precautions, and respiratory needs as applicable
Complete accurate, timely documentation including shift flow sheets, progress notes, and EVV records
Communicate changes in client condition promptly to the Clinical Director and family/support team
Respond to clinical emergencies following established protocols
Participate in care plan reviews and updates
Maintain compliance with all Oregon Health Authority and ODDS regulations
Requirements:
Active, unrestricted Oregon RN license
Current CPR/BLS certification
Experience with pediatric or developmental disabilities nursing preferred
Experience with G-tube, seizure, and/or respiratory care preferred
Reliable transportation and ability to travel to client's home
Strong documentation and communication skills
Ability to pass background check and required agency onboarding
Availability for full-time scheduled shifts
